Start taking an active role in managing your mental health care with My Mental Health Crisis Plan. This tool allows you to develop a psychiatric advance directive (PAD) or mental health advance directive to outline your preferences in the event of a mental health crisis, enabling you to guide your care effectively.

 <li>CREATE your personalized PAD</li>
 <li>SELECT individuals to share your PAD with, such as family or treatment providers</li>
 <li>STORE your PAD securely within the app, accessible from anywhere</li>
 <li>EDIT and SAVE your PAD whenever your preferences change</li>
</ul>

This resource also includes guidance on establishing legally binding instructions for your preferences. By creating a PAD, you can:

<ul>
 <li>Play a more active role in your mental health treatment</li>
 <li>Be better prepared for potential crises</li>
 <li>Determine trusted contacts who can act on your behalf</li>
 <li>Communicate your choices with medical professionals and your care team</li>
</ul>

Define the decisions you wish to be made during a mental health crisis, including preferences for emergency treatments, medications, physicians, treatment facilities, and who should serve as your decision-maker.
